Policy & Procedures 

 

FIRST APPOINTMENTS-

Please arrive for your appointment 15 minutes prior to starting time.  You will be asked to complete the new client forms and review the policy and procedures.  Your therapist will review with you what to expect during your massage and answer any questions you may have.  Please arrive early as this allows for the most comprehensive screening and creates a relaxed start to your massage.

PAYMENT-

Major credit cards, gift certificates, cash and checks are accepted. (Ask your therapist who to make your check payable to please).

GRATUITIES-

Gratuities are not included in treatment fees.  All gratuities are appreciated and accepted by the therapists.

GIFT CERTIFICATES-

Gift certificates are available and transferable as well as redeemable up to expiration date.

INSURANCE-

Insurance is not accepted at this time.  Please let your therapist know if you will need a receipt and or copies of records to submit for reimbursement prior to your service.  It is important to note a physician referral demonstrating medical necessity is required for insurance reimbursement regardless of who submits bills.

EXISTING AND NEW MEDICAL CONDITIONS-

               It is very important to inform your practitioner of all existing and new medical conditions, including all prescriptions and medications you are using. Massage therapist do not diagnose or prescribe for medical conditions.  If your therapist thinks medical attention is needed or treatment for a specific medical condition is needed she will ask for your doctor's permission and supervision before treatment can begin.  Additionally, the therapist may refer you for diagnosis and refuse to treat without recommendations from your physician.  Furthermore, it is the clients responsibility to provide written permission from the physician, chiropractor, physical therapist, etc.  If for any reason your therapist feels she is unable to treat your condition she may refer you to another professional. 

              We reserve the right to refuse service for any reason, excluding; age, race, religion and sexual preferences.  All clients will be treated fairly and respectfully at all times.  Melinda's Mindful Minutes adheres to the strict scope of practice as a licensed massage therapist. Massage therapists provide therapeutic massage and spa treatments.  We abide by a high standard of ethics and work with a strict, no tolerance to misconduct policy.  Request for sexual activity by anyone will not be tolerated and will be viewed as solicitation. All illicit behavior will be reported to the proper authorities.  We reserve the right to refuse and end any session due to violation of client therapist trust.
